Family Membership
One simple monthly investment, no matter how many kids you have.
No copays. No rushed visits. Just thoughtful, personalized care - when you need it.
Enrollment fee $50 per family. Required for all new memberships and covers account creation and medical record review.
For past members who cancel and later wish to re-enroll: First time re-enrollment fee $200. Second time re-enrollment fee is $500.
A 10% discount is available for patients who pay for 12 months.
Membership Fee Inclusions
Membership includes unlimited visits for well-child checkups, sick visits, and follow-up care for chronic conditions.
